Families receive one clear, consistent update from their consultant instead of conflicting messages from multiple sources.
We understand the language of hospitals, social care, and care agencies, translating complex information and ensuring no detail is missed while keeping families fully informed.

Care Discovery Service:
Initial consultation via telephone, Zoom, or in person (depending on location), where you speak with a consultant about the type of care you or your loved one may need.
The consultant then collaborates with you and your medical practitioners to create detailed ‘Care Plan’ and ‘About Me’ documents. Once approved, the consultant researches suitable care options in your area that match your specific requirements.
A follow‑up meeting takes place after the initial risk assessment. During this meeting, the consultant provides a personalised list of healthcare professionals and organisations best suited to deliver the most appropriate care package for you.
A concluding meeting is held after the care package has been in place, allowing the consultant to review how well the support is working and to identify any changes or additional needs that may have arisen since the initial consultation.
Care Transition Service:
This package includes everything provided in the Care Discovery Service, with the added benefit of your consultant arranging meetings with care providers on your behalf and attending the interviews alongside you.
These interviews may take place by telephone, online, or in person, depending on what is most suitable.
Following the interviews, the consultant prepares a clear summary of each care option, outlining the key benefits and associated costs to support your decision‑making.
Returning Clients
Healthcare needs often evolve gradually, and this fourth package is designed for clients returning to Maxel Care Consultancy for updated clinical guidance as their condition or circumstances change.
Your consultant reviews your concerns and the adjustments you wish to make to your current care. With your consent—and that of your medical practitioners—they complete an updated clinical risk assessment and revise your care plan, explaining the clinical and environmental reasons for each recommended amendment as your health status or living arrangements have progressed.
