Our dedicated care liaison service solves family’s stress, confusion, and time drain during a crisis by being the Single Point of Contact for your loved one while they are in hospital. It's not just administration; it's expert knowledge, management and peace of mind.

When families are overwhelmed, don't know the system, waste time on hold, and risk missing crucial information, often while dealing with emotional distress, we step in and act as their single, knowledgeable point of contact and professional advocate, managing the entire process from hospital to home/care facility.

Our daily proactive management takes on all the calls, updates, and liaisons, at any time of the day and night. This immediacy and responsiveness provides you with the highest commitment to care.

The family receive one clear, concise update from their consultant, instead of many conflicting messages from various sources.

We know the medical language of hospitals, social care and agencies and we can translate this for you, ensuring no detail is overlooked and the best care is secured, whilst keeping the family fully informed.

This package provides a smooth transition with crisis-to-care continuity, ensuring the next steps are planned, funded and ready before hospital discharge, preventing last minute panic.

The peace of mind this dedicated care liaison service provides allows the family to focus on supporting their loved one, knowing the logistics and advocacy are handled by a dedicated professional.

Capped at £180 per hour

Care Discovery Service - Package 2:

Initial consultation via telephone, zoom or in person (depending on location) where you will speak with a consultant about the care you feel you or a loved one requires.

The consultant then works with you and your medical practitioners to create comprehensive ‘Care Plan’ and ‘About Me’ documents. Once these are approved, the consultant researches care in your area that suits your requirements.

Follow up meeting after the initial risk assessment, where the consultant provides you with a personalised list of helpful healthcare professionals and companies who can provide you with the most appropriate health care package to suit you.

Conclusion meeting after the package has been followed, to check that the healthcare is working for the individual and if there is anything else that has changed since the initial consultation.

Care Transition Service - Package 3:

Everything that Package 2 provides, plus the consultant organising the meetings with the care companies on your behalf and interviewing them with you.

These interviews can include telephone, online or in person meetings.

The consultant then provides you with a summary of each care option, detailing the benefits and costs involved.

Returning Clients

Healthcare needs develop gradually, and this fourth care package is for those clients returning to Maxel Care Consultancy for advice to cover the changes that have happened over time.

Your consultant listens to your concerns and the modifications you feel you would like to make to your care. With your and your medical practitioner’s consent, the consultant compiles a revised risk assessment and care package and an explanation of why the care requires amendments due to health or living arrangements altering over the course of your existing health care.
